…..The falling products from the top shelf caused the other three shelves to collapse and this led to the entire shelving rack coming away from the wall and collapsing. John O’Reilly had been stacking the lower shelves and was aware of Keith climbing but he thought no big deal of it. He was crouching down when the top shelf buckled and by the time he looked up it was too late for him to get up. The shelving and the products, some of them heavy, rained on top of him and he was flattened.
